HOURLY SALARY: Min. (1) $24.21 – Max. (12) $32.32

SUMMARY OF THE POSITION

Person who, agrees to attend all the necessary trainings offered by the organization or within the limits of her/his skills and training, does work that is similar in nature and related to that of a Primary Care Community Representative worker.

SPECIFIC FUNCTIONS

The Primary care community representative works under the supervision of Chishaayiyuu and Current Services coordinator in the CMC is responsible for providing culturally sensitive health and psychosocial awareness to Chishaayiyuu adults meeting them in their settings.

  • Acts as a connecting link between CBHSSJB health professionals and community members.
  • Acts as an educator, an advocate, a leader and a health promoter providing individuals with information and needed support.
  • Works in collaboration with health and social services professionals in planning, developing and implementing health programs and promoting healthy lifestyle to children, adults and Elders while raising awareness of key health issues affecting the Cree population.
  • Plans with members of the health team and social services team the local implementation of specific regionally applied C.B.H.S.S.J.B. Community Health Promotion and Education programs and carries out the health promotion and educational activities.
  • Develops and implements methods to evaluate community members’ progress and to make any recommendations for modification.
  • Monitors his own performance and identifies the needs for further support and direction.
  • Participates in weekly team meetings for the purpose of planning and revising activity schedules, discussing issues of concern, and sharing new ideas.
  • Provides information to community members on the pathway of care and social services.
  • Provides support, training or guidance within the framework of his functions to other staff members, health care providers, or to those supporting the participants’ quality of life.
  • Collaborates and communicates with all other staff, for the benefit of the users and/or the community members.
  • Ensures progress notes and other documentation is done after the service has been provided.
  • Respects the practices, conduct, confidentiality and ethics expected of employees towards community members, visitors and other employees.
  • Ensures his own responsibilities according to the organization’s programs, policies, procedures and Program Manuals, and policies regarding access to patient files, in order to provide quality services.
  • Provides good “customer service” in an agreeable manner to all users and other staff, in order to foster organizational functioning and, indirectly, quality services to the population.

     

  • Carries out his role within the policy principles and guidelines of: Miyupimaatisiiun paradigms and practices, and; collaborative interdisciplinary team-work and communications, for services and programs implementation within a seamless integrated circle (continuum) of care, centred on the community residents.
  • Respects the practices, conduct and confidentiality expected of employees towards community members, visitors and other employees.
  • Ensures the carrying out of her or his duties according to the organization’s procedures and guidelines in order to provide quality services.
  • Carries out other tasks usually assigned to a PCCR (Primary Care Community representative) upon the request of his supervisor.

REQUIREMENTS

EDUCATION:

  • High School diploma.

EXPERIENCE:

  • Acquired 1 year pertinent experience.

KNOWLEDGE AND ABILITIES

  • Basic understanding of the services offered by the CMC Chishaayiyuu and Current Services and be aware of the other services available in the community and surrounding area;
  • Ability to work with the targeted clientele;
  • Ability to write short reports; (written exam);
  • Capacity to establish interpersonal relations;
  • Capacity to work in a multi-disciplinary team;
  • Excellent listening communication skills;
  • Excellent critical thinking, organizational and decision-making skills;
  • Autonomous, likes to take initiative, flexible, discrete and empathetic;
  • Sense of accountability, resourcefulness and vigilance.

LANGUAGE

  • Fluent in Cree and English;
  • Fluency in French is an asset.
